If a non-formulary, brand name drug is pre-authorized, you pay the brand copay of $10. If a non-formulary brand name drug is  pre-authorized, but if you feel you need the brand name drug, you pay the generic copay of $5 plus the difference in cost between the brand name drug and the form ulary generic.

Mental health drugs are not covered.
