Bay Health Plan is a community-sponsored program for eligible residents in Bay County. You may qualify for Bay Health Plan if your live in Bay County, Michigan, have no health insurance, and meet income requirements. You must complete an eligibility form at the Bay County Health Department or participating enrollment site. Please call Bay Health Plan at Bay County Health Department, (989) 895-4008 to enroll.

This is not insurance. Bay Health Plan provides limited services for doctor visits, prescriptions, lab tests and x-rays. Bay Health Plan does not pay for hospital or other inpatient services. If you receive services that are not covered through Bay Health Plan, you will be responsible for payment.
